Join us for a thoughtful and empowering community conversation during Advance Care Planning Month.
This one-hour session is designed for community members who want to better understand how to plan ahead in ways that honor dignity, choice, and personal values. We welcome individuals with disabilities, older adults, caregivers, family members, and anyone supporting a loved one. Healthcare professionals such as nurses, social workers, case managers, and others working in the field are also encouraged to attend and deepen their understanding of advance care planning.

- Tools available in Maryland, including Advance Directives, Five Wishes, conversation guides, and information about the Medical Orders for Life-Sustaining Treatment (MOLST) form
- How to begin conversations with loved ones
- Where to turn for support

Anna Moretti, RN, JD, ANP-C is an advanced practice nurse, certified grief educator, health and well-being coach, and legal professional with more than 20 years of experience across diverse healthcare settings. Her work bridges clinical care, legal insight, ethics, and coaching to support individuals and families navigating serious illness, complex health conditions, and life transitions.
Earlier in her career, Anna served as the Legal Director of Choice In Dying, Inc., the national organization that worked to establish advance directives in every state. Her work contributed to the development and expansion of advance care planning tools that are now foundational to patient autonomy across the country.
With training in both nursing and law, Anna brings deep expertise in the legal, ethical, and practical dimensions of advance care planning. She remains actively engaged in clinical ethics through her participation in the Maryland Ethics Network and is a trainer for ELNEC (End-of-Life Nursing Education Consortium), which promotes a unified, high-quality approach to educating healthcare providers in end-of-life care.
As a health and well-being coach, Anna partners with individuals and families to clarify their values, identify trusted decision-makers, and take meaningful steps toward protecting their voice and autonomy. Drawing on her extensive experience in serious illness care, she understands the physical, emotional, social, and ethical dimensions that affect patients and families. Her approach is grounded in compassion, dignity, and empowerment — helping individuals take an active role in their health journey at every stage of life.
Anna is passionate about helping people engage in thoughtful conversations about what matters most and ensuring that their care preferences reflect their values and strengths.
