Through our Supports Planning Services (SPS) program, SCI partners with people to navigate complex systems such as Medicaid and long-term services and supports.
Our holistic approach is rooted in respect for each person’s dignity, independence, and right to make informed, self-directed decisions about their life.

We believe that every person should choose the supports and care that suits their individual vision of a good life. Our Supports Planners work with you to understand what that means to you and can help you access home and community-based services (HCBS), which are administered by the Maryland Department of Health, Office of Long Term Services and Supports.

Living Safely at Home
After a stroke and a long stay in a nursing center, a Prince George’s County man longed to return home to his family. With help from his Supports Planner, he enrolled in the Community Options (CO) Waiver to receive 40 hours of personal assistance each week, along with nurse monitoring through the local health department. Thanks to these supports, he was able to return home to his wife and is once again surrounded by the family who cherish his kindhearted, outgoing spirit.

Gaining Independence
When her aging mother passed away, a young woman with a developmental disability was left vulnerable—homeless and financially exploited. Her Supports Planner quickly connected her to the Community First Choice (CFC) program, helping her secure emergency, then permanent housing, along with vital documents like her birth certificate and Social Security card. Once settled, she received personal care services, home-delivered meals, and coaching on everyday tasks—support that has helped her build stability and independence.

Caring for Your Children
A Maryland woman with Cerebral Palsy, who had undergone many surgeries since childhood, was overjoyed to be expecting her first child. But pregnancy complications made it difficult to stand or walk. Her Supports Planner arranged meal preparation, personal assistance, and daily living supports, giving her more time to care for her baby. Together, they also secured a more accessible apartment with key environmental adaptations for her growing family.

Struggling with mental illness and the loss of several loved ones, a Baltimore woman in her 40s became isolated at home and had not received medical care in years. Her Supports Planner built trust by listening, helping her schedule appointments, and easing her fears about communicating with doctors. Through the Community First Choice (CFC) program, home modifications—including grab bars, stair railings, and an accessible tub—made her home safer and allowed her to remain independent.
