Targeted Case Management (TCM) Services help people with complex needs connect to important medical, social, educational, and other support resources. A skilled case manager works closely with the person, their family, and other professionals to create and carry out a plan that meets the person’s needs. This includes assessing the situation, planning next steps, coordinating services, and advocating for the person’s best interests. The goal is to make sure people get the care and support they need for their health and well-being, while also improving their overall experience, ensuring safety and quality, and making the process as effective and affordable as possible.

Case management happens in many sectors. Service Coordination, Inc. (SCI) provides case management support through two programs:

  • Coordination of Community Services (CCS) for people with intellectual and developmental disabilities and their families, through the Maryland Developmental Disabilities Administration (DDA). CCS Coordinators help individuals plan for the future, navigate systems, and reach personal goals.
  • Support Planning Services (SPS) to individuals with various medical needs through the Maryland Department of Health’s Long Term Support Services (LTSS) program. Support Planners work closely with individuals, families, and caregivers to create and implement a plan that promotes health, independence, and quality of life.

As an independent non-profit, SCI offers conflict-free case management support, meaning we are a separate entity from those providing services. In some cases, a potential conflict may exist if the case management provider is also offering the referred services.  SCI’s expertise is in understanding an individual’s goals and identifying resources which the individual may choose to engage.

A case manager is more than just a guide—they’re your advocate and partner, helping you navigate complex systems, access essential services, and take meaningful steps toward the life you envision.

While many resources are publicly available, finding and coordinating them can be overwhelming. That’s where we come in. With SCI, you don’t have to figure it all out alone. Our trained case managers simplify the process so you can focus on what truly matters: living the life you choose.  If you qualify for Medicaid, our case management services are available to you at no cost.

Your Coordinator of Community Services (CCS) or Supports Planner will help you locate the services you qualify for and direct you to the application for those agencies. Services being provided are determined by eligibility and need.

Coordination of Community Services (CCS) provides case management support for people with intellectual and developmental disabilities and their families, through the Maryland Developmental Disabilities Administration (DDA).  CCS Coordinators help individuals plan for the future, navigate systems, and reach personal goals.

Once your eligibility is determined and you have a plan, your CCS can help you navigate the system to access home and community-based services such as:

    • Residential / Personal Support Services
    • Day / Employment Services
    • Support Services

Our services include:

      • Eligibility assessment for DDA services
      • Development of an Individualized Person-Centered Plan
      • Referral to various services and resources in the community
      • Transitioning Youth Services
      • Transition Coordination Services (from institution to community)
      • Obtaining funding for DDA services
      • Monitoring and follow-up

Supports Planning Services (SPS) provides case management support to individuals with various medical needs through the Maryland Department of Health’s Long Term Support Services (LTSS) program. Support Planners work closely with individuals, families, and caregivers to create and implement a plan that promotes health, independence, and quality of life

Once your eligibility is determined and you have a plan, your Supports Planner can help you navigate the system to access a range of home and community-based services, tailored to your needs and based on your eligibility.

Our services include:

    • Eligibility assessment for LTSS services
    • Development of an Individualized Person Centered Plan
    • Referral to various services and resources in the community
    • Monitoring and follow-up

Yes!  SCI employs Team Members who are fluent in a variety of languages, including American Sign Language (ASL) and Spanish. We take a person-centered approach in supporting individuals, working collaboratively with them and their families to identify and address their unique communication needs and preferences. This could be achieved through the use of interpreters, assistive technology, and other tailored solutions.

In addition to English, ASL and Spanish, over the past year, we have supported people who speak more than 20 different languages, including Korean, Farsi and Urdu. We encourage you to reach out to us directly to discuss your needs.
