At Service Coordination, we believe in putting people first and supporting individuals of all ages and abilities in living life on their own terms.
That’s why we’re proud to incorporate the Charting the LifeCourse (CtLC) framework, a person-centered approach that helps people envision and achieve the lives they want, into our trainings and programs.
David Bright, Supports Planning Services Supervisor, has been instrumental in integrating the CtLC framework into our Supports Planning Services program. His leadership has helped more than 230 team members adopt the tools, embrace the values, and incorporate them into their daily work.
David’s commitment to person-centered practices made him a key leader in Service Coordination’s integration of the CtLC framework from the very beginning. David took a proactive approach, launching pilot CtLC programs, during which he educated and guided his peers through best practices, and hosting weekly “CtLC Office Hours,” where he made himself available to address barriers, answer questions, and provide a space for sharing ideas and feedback.
David’s dedication to CtLC did not go unnoticed. Recognizing his efforts, Ginny Jones, Vice President of Supports Planning Services, nominated David for the Innovations in Person-Centered Practices in Case Management Award, which was to be presented at the 2025 Charting the LifeCourse Showcase.
“I felt this award was perfect for David ,” Ginny says. “I chose to nominate him due to his creativity, hard work, and unwavering support in implementing CtLC within the SPS Program.”
Ginny wasn’t alone; David was chosen as the recipient of the award, which was presented to him at the showcase in Spring 2025 in Kansas City, MO.
“Being recognized for the work we’ve accomplished over the past couple of years was truly special,” David recalls. “Receiving this award confirms we’re making significant strides in enhancing our person-centered practices.”
David maintains a steadfast commitment to person-centered practices, and to providing the best care possible to the individuals supported by Service Coordination, which he says naturally led him to become a strong advocate for the CtLC framework.
“This approach recognizes the unique vision that each person has for their life, ensuring that support is tailored to align with their personal preferences,” he says. “By integrating these elements into the decision-making process, we create an environment where individuals feel valued and empowered.”
The CtLC framework doesn’t just make the people we support feel in control. It empowers them to be in control.
“We’re not only listening to them,” David says. “We’re actively involving them in choices that affect their lives.”
As Service Coordination continues to grow, we remain committed to putting people first. With the CtLC framework at the heart of our work, and team members like David leading the way, we’re helping individuals across Maryland build the lives they choose on their own terms.
