Instead of starting with paperwork, we start with questions like:

These conversations help shape a plan that’s unique to you. Maybe it means joining a local community group, eating healthier, or finding time for hobbies. Not every goal ties directly to state-provided services, and that’s okay. Your Supports Planner can research resources and walk beside you as you pursue them.
We have dedicated registered nurses who provide holistic educational material that can be shared with you, such as diabetes management, falls prevention, and other health and safety topics. Your Supports Planner will share applicable information during monthly monitoring calls and scheduled home visits.
Example: One SCI participant wanted to focus on healthier eating after surgery. Their Supports Planner worked with a doctor and connected them to meal-delivery options, ensuring the plan was realistic and sustainable.
